NOTE: If you are completing this form AFTER Friday August 7th, 2026 at 12:01AM, you will NOT be able to receive a vote by mail ballot for the August 18th primary election but you will be enrolled for any future elections. You can also vote in person by going to an early voting location in your county OR vote in person on Tuesday August 18th at your assigned polling location.
To vote by mail, select your county and fill out the form. You will then be redirected to your county’s official vote by mail request page, which you must complete in order to vote by mail.
The deadline to request that a ballot be mailed is no later than 5 p.m. on the 12th day before the election. A Supervisor of Elections must mail the ballot out within 2 business days after a request but no later than the 10th day before election day.
